Official Notice – December 1, 2022:

Patients treated by Eastern Niagara Hospital (ENH) may obtain their ENH medical records the following two ways:

1. By obtaining a copy from ENH: Medical records for adults treated by ENH since 2016 and pediatric patients treated by ENH since 2001 may be obtained through November 30, 2023 by submitting a completed Authorization Release Form to ENH. The Authorization Release Form is available at: www.enhs.org and in the lobbies of the main hospital (521 East Ave, Lockport) and Urgent Care (5875 S. Transit Rd, Lockport). The completed Form may be faxed to: 716-514-5786, mailed or hand delivered in a sealed envelope to: Eastern Niagara Hospital, attn: Medical Records, 521 East Ave, Lockport, NY 14094. After November 30, 2023, ENH patient medical records will be safely and confidentially destroyed, and the ENH Patient Portal will be disabled [Pursuant to 11 U.S. Code §351].

2. Through your Primary Care Provider: To the extent ENH and other providers have shared records with HEALTHeLINK^TM, those records will continue to be accessible by your Western New York Primary Care Provider. HEALTHeLINK^TM is a community-based non-profit organization that maintains a secure and reliable infrastructure for the timely and accurate electronic exchange of clinical information among health care providers and others involved in the delivery of health care services in Western New York.
